[ARCHIVED] City Employees - Bringing Your Own Lab Results to Health Screening

Questions have been raised by some employees regarding participation in the health risk assessment process and whether they can have their labs (blood) drawn by their own medical care provider.

This has been carefully considered, and we will follow the suggestion of MMG Occupational Medicine and allow employees who have had labs drawn to earn the points for the Health Risk Assessment and participate in the process by signing up for the health risk assessment and bringing a copy of their lab work with them.

The lab tests must be fairly recent, having been run within the last 60 days. Also, at a minimum, the following tests would need to be included:

Total cholesterol
HDL
LDL
Triglycerides
Glucose

Employees bringing in their lab results would still participate in the other portions of the assessment (height, weight, blood pressure, waist, and hip measurements), including the questionnaire portion.

Lab work must be an official copy (or copy of the official lab report), provided to you by your provider, and show date labs were completed. The report must also show where the lab work was completed.
