HSA contributions will be pro-rated for completion of the Wellness Program. Maximum points are 500 for employees under a Single health plan and 1,000 for those who carry a Family plan. Points will be earned upon completion of parts of the program.
Points will be earned based on the health plan you carry (single plan or family plan) at the time you complete each component of the Wellness Program. The five components of the program are listed below.
Contributions will be pro-rated for partial completion of the program. Maximum points for those employees under a single plan are 500 (which equals a $500 HSA contribution). Maximum points for employees carrying a family plan are 1,000 ($1,000 HSA contribution).

Initial Assessment: 100 Single plan, 200 Family Plan
Biometric Screen and Questionnaire Completed: 100 Single plan, 200 Family Plan
1st Follow Up Assessment: 100 Single plan, 200 Family Plan
2nd Follow Up Assessment: 100 Single plan, 200 Family Plan
Final Assessment: 100 Single plan, 200 Family Plan
Points earned in 2014 will count towards an HSA contribution in 2015. The contribution will likely be placed in your account in early to mid-January, 2015. In order to receive the HSA contribution, the employee must be eligible to have an HSA (i.e. must have coverage on a high deductible health plan and cannot participate in a full FSA Plan). Employees considering retirement or separation of employment who separate from City employment before the HSA contribution is made in early 2015 will not be eligible to receive an HSA contribution, even if they have completed part or all of the program.
